New: forms available online for clients

by Veronica Alvarez & Dennis Tivey

In an effort to improve our clients’ experience and improve our online resources, we have added many of the most frequently used forms for the CalWORKs, CalFresh, and Medi-Cal programs to our public-facing website, sbcDSS.org! Please spread the word about this new self-service feature with your clients.

Clients can navigate to the forms by clicking More > Frequently Used Forms. Or you can give them this direct link: http://countyofsb.org/dss/services/forms.sbc. The forms are organized alphabetically, and nearly all of them are available in both English and Spanish.

Most have been converted into PDF fillable forms that desktop/laptop computer users can type right into if desired. Most desktop/laptop browsers, such as Chrome, permit this without downloading anything extra.

Most clients will print the form, sign it, snap a photo of it and send us the photo via OCTOPUS (DSSoctopus.com). Easy!

What if the client doesn't have a printer, or only has a mobile device? If that's the case, and the client is not tech-savvy, then it's best to just send them a paper form. But if they are tech-savvy and up for a learning adventure, then it is also possible to complete, sign, and send us a form without a printer or pen...read on to learn how.

  1. Download the appropriate free Adobe application: Acrobat Reader for desktop/laptop, or Fill & Sign for mobile.
  2. Open the form in the application, and fill out the form. Note: Fillable forms often don't show up as fillable on mobile, but Fill & Sign overcomes this with a feature that can automatically add new fillable fields wherever a blank or checkbox appears on the form.
  3. Use the the application's Signature function to sign the form. This feature lets you draw your signature with your finger if your device has a touchscreen, or with a mouse if it doesn't.
  4. Use the application's Print to PDF function to save the completed and signed form. Note: in Acrobat the Save function will just save the blank form, so you need to use the Print to PDF function instead.
  5. Go to OCTOPUS, answer the questions, attach the PDF file, and send.
